The Orange County, Nurse-Family Partnership is the local site
of the national Nurse-Family Partnership program based in
Denver, Colorado. It is an intensive home visitation program for
teens and young women under 24 years old, who meet the
eligibility requirements. A public health nurse will visit the
first-time mother in her home or other location throughout the
pregnancy and until the child’s second birthday. She will assist
the new mother and her family by providing education and support
in the areas of prenatal care, nutrition, childbirth,
breastfeeding, child development, child safety, and setting and
reaching personal goals.

To foster healthier pregnancies
To improve the health and
development of children
To encourage self-sufficiency
Program Eligibility:
Under 24 years old, and
Less than 28 weeks pregnant*, and
First time pregnant and
parenting, and
Eligible for Medi-Cal, CalOptima
or emergency/pregnancy related Medi-Cal, and
Desire to be involved in this
program
(* The nurse must be able to make a
home visit before the 28th week of pregnancy. Please refer as
early in the pregnancy as possible.)
Home Visit Schedule: (Once the
mother decides to enroll in the program)
Weekly for the first 4 weeks,
Every other week until the child
is born,
Weekly, again, for 6 weeks,
Every other week until the child
is 21 months old,
Monthly until the child is 2
years old
