Maintain, expand and assess the countywide infrastructure - both public and private - providing health care to the County’s low income and uninsured infants, children, adolescents and pregnant women through the:

• Child Health and Disability Prevention (CHDP) provider network

• CHDP Diagnosis and Treatment provider network

• Comprehensive Perinatal Services Program (CPSP) provider network

• Prenatal Care Guidance (PCG) provider network

Facilitate and assess immunization infrastructure at public health clinics, community clinics, schools and day care centers through the:

• Immunization Assistance Project (IAP)

Provide direct case management services to assure linkage to health care to: lead burdened children and their families; pregnant and/or parenting teens and their siblings; Public Health Maternal Clinic clients; Public Health Pregnancy Testing Clinic clients; and women with Hepatitis B, their babies and household contacts through:

• Childhood Lead Poisoning Prevention Program (CLPPP)

• Adolescent Family Life Program (AFLP)

• Cal-Learn Program

• Outreach Worker staff

• IAP Perinatal Hepatitis B Project

Raise awareness in the community of the importance of: a medical home; ongoing preventive health care; timely and complete immunizations; family planning; and early and ongoing prenatal care – including access to such services through the education, marketing and outreach components of:

• CHDP     

• IAP

• CLPPP

• Perinatal Outreach and Education

• Family Planning Education

• Health Referral Toll Free Line
