4. How can someone find out about travel immunizations?
Call (714) 834-8444 for recorded information about travel immunizations, or call (800) 564-8448 for directions to the centrally located Santa Ana clinic where the travel immunizations are given.

6. What is CHDP?
CHDP (Child Health and Disability Prevention) is a State funded program that provides free periodic preventive health services known as well-child check-ups or physicals, or health assessments to income eligible children and teens up to age 19, and to Medi-Cal eligible children to age 21. Income eligible children receive services through the Public Health Clinics and the private sector. Medi-Cal eligible children receive services through
health plans.

7. What is included in a CHDP check-up? The check-up includes:

Health history and physical exam

Dental, nutrition and developmental assessments

Immunizations
-
Vision and hearing screenings

TB, lead, anemia, and other screenings, when needed

Health education

8. Who is eligible for a CHDP check-up?

Children from birth to 21 years of age who are on or eligible for Medi-Cal
-
Children from birth to 19 years of age, who do not have health insurance, are not on Medi-Cal and are income eligible

9. How can
parents find a CHDP provider?
Parents may get a referral by calling the Health Referral Line at (800) 564-8448, Monday through Friday, 8 a.m. to 5 p.m. Phone staff can refer to private physicians and clinics near the caller’s home.

10. What is Cal-Learn?
Cal-Learn helps pregnant and parenting teens complete high school or obtain their GED. Cash aid or Temporary Assistance for Needy Families "TANF" eligible teens (male or female) are assigned a social worker who help them enroll in and complete school, and obtain support services such as child care and transportation. The social worker also assists the teens get other resources such as prenatal and postpartum care, well-baby care and immunizations, parenting classes, birth control, counseling, housing, vocational goal development and training.

11. How can a teen enroll in Cal-Learn?
Pregnant and parenting teens receiving cash aid or “TANF” are eligible and are referred to the program by their Social Services eligibility worker. Teens who do not qualify or who are not receiving cash aid are referred to the related Adolescent Family Life Program (AFLP). The AFLP phone number is 714-834-7866.

12. Does Cal-Learn help with child care?
Yes. The Cal-Learn program pays child care as long as the mother is enrolled in and attending high school or a GED program full time.

13. What is the Health Referral Line?
The Health Referral Line (HRL) is a toll-free telephone service. Staff answering the phones provide the public with referrals to:
The HRL also gives referrals to other health-related programs, services and community resources.

14. What do clinic services cost?
Some services are free to eligible families. Other services are provided on a sliding scale that is based on the family size and income. Families or individuals who do not qualify are given referrals to three physicians or clinics nearest their home. For travel immunizations, clients pay a service fee and a fee for the vaccine or medication they receive.

15. Are immunizations free for children?
Yes. Immunizations are free for all children under the age of eighteen.

16. What services are available from the family planning clinics?
Services are available to both women and men who need family planning services, lack health insurance, and qualify financially.
Female clients receive:

Pap smear,

counseling and educational materials,

contraceptives (when appropriate),

treatment of infections, and when needed,

follow-up and referrals
Male clients may receive:

18. What is Adolescent Family Life Program (UPPBEAT)?
Adolescent Family Life Program (AFLP) helps pregnant and parenting teens complete high school or obtain their GED.
