Guide to School Immunization Requirements
Maternal, Child and Adolescent Health (MCAH)
The State of California requires schools,
childcare centers and family child care homes to assess and
maintain immunization records. Reports on the status of
kindergarten students and all children over age two enrolled in
day care are completed annually. State regulations specify the
vaccines required and establish the proper procedures for
collecting, assessing, reporting and maintaining this information.
Requirements for School Entry
Grades K - 12

- 3 doses if at least one dose was given after the second birthday for children age 7 years and older

| MMR |
For kindergarten and seventh grade entry:

2 doses both given on or after the first birthday
(One dose must be MMR and one dose may be any other measles-containing vaccine.)
For grades 1-6 and grades 8-12:

1 dose of MMR on or after the first birthday
(Mumps vaccine is not required after the age of 7.)

Varicella

For kindergarten entry (as of July 1, 2001)
- 1 dose on or after the first birthday or health care provider-documented disease history
