Proof that you live in Orange County. For example, current utility bill, California motor vehicle registration with your current address in Orange County, or mortgage payment receipt. P.O. Box address is not acceptable for address verification.
Fees (Non Medi-Cal: $84.00; qualified Medi-Cal participants will receive a 50% reduction in fee: $42.00). Payments can be made by cash or money order. If you choose to pay by cash, exact change is appreciated. Application fees are non-refundable.
