An interagency agreement exists between the Social Services Agency (SSA) and the Health Care Agency (HCA) to provide Child Health and Disability Prevention (CHDP) Program annual training to Eligibility staff and Supervisors.
During the months of January through July 30, 2013 all Eligibility staff will be required to view an online training presentation about the CHDP Program. The training is available online starting June 12, 2013, and all Eligibility staff must complete the training no later than August 13, 2013.
Staff should set aside approximately 45 minutes to complete the training.

All three steps, Pre Training, Training, and Post Training require a form be filled out and submitted online and are accessible through the link below.
Make sure your name is consistent on all three forms, or you may not receive credit for completion.
After the training, you will receive a certificate. It should be signed, collected, and controlled by the unit Supervisors to ensure that all Eligibility staff has completed the training as of August 13, 2013.
