|
Burial permits are used for burial, cremation,
scattering or disinterment. Burial permits
may be obtained through any licensed funeral director or by
applying in person at the Birth and Death Registration office
located at:

To obtain a new
burial permit, bring either a copy of the original burial
permit, transit burial permit, or death certificate. If you
do not have a copy of the original forms, you will be
required to complete a "Disinterment Form" prior to
receiving the new burial permit.

To verify the
identity of the person requesting the new burial permit and
to ensure that this person has the right to control
disposition of the remains, Birth and Death
-
Registration staff
will ask the requestor for valid photo identification.
The fee for a burial
permit is $11.00 (Effective January
1, 2005) and may be purchased from
Birth and Death Registration between the hours of 8:00 - 11:00
a.m. and 1:00 - 4:00 p.m. Monday through Friday, except on
holidays.
