financial or residential requirements are necessary for physical
or occupational therapy services
CCS provides medical case management,
physical/occupational therapy services, and financial
assistances for children with certain health care needs.
Anyone may refer a child to CCS by completing a
referral form, providing adequate medical reports to determine
eligibility and completing a program application.
Care is provided by CCS approved paneled
physicians in the State of California.
