Public Health Nursing
Public Health Community Nursing provides nursing
services in the home and community with a focus on communicable
disease investigation and maternal-child health for high risk
adolescents, women and their children.

The Public Health Community Nursing
Mission is to improve the health of our community by providing
quality Public Health Community Nursing services.
Services include:
Communicable Disease Investigation:

Investigation of selected
reportable communicable diseases and cases found in the
community

Case management of persons who
require laboratory testing in sensitive occupations/situation to
prevent spread of the disease

Education regarding transmission
and prevention of communicable diseases

Referral for follow-up as needed

support for perinatal loss/Sudden Infant Death Syndrome (SIDS)
Project Connections Family Resource Center (FRC) - Health Access:
Public Health Nurses provide
consultation to Family Resource Center Health Access Teams and home
visitation to families referred to the FRCs. The Health Access goal
is to link families to a health care home/immunization resource and
funding for both. The Public Health Nurse and the Promotoras (health
promoters) team up to provide in-home assessments, interventions and
case management to meet family needs.
