Public Health Community Nursing
Public Health Nursing

Public Health Community Nursing provides nursing services in the home and community with a focus on communicable disease investigation and maternal-child health for high risk adolescents, women and their children.

The Public Health Community Nursing Mission is to improve the health of our community by providing quality Public Health Community Nursing services.

Services include:

Communicable Disease Investigation:

  • Investigation of selected reportable communicable diseases and cases found in the community

  • Case management of persons who require laboratory testing in sensitive occupations/situation to prevent spread of the disease

  • Education regarding transmission and prevention of communicable diseases

  • Referral for follow-up as needed

Infant and Child Health:

  • Assessment, health teaching, referral and follow-up of:

    • Prematurity

    • Perinatal substance exposure

    • Developmental delays

    • Sick child

    • Congenital conditions

  • Emphasizing:

    • Developmental screening tests

    • Parent/child interaction assessments

    • Preparation for hospital discharge

    • Facilitation of family support system

    • Use of special equipment

    • Understanding of medical conditions

    • Follow-up of physician’s recommendations

    • Promotion of healthy practices, including immunizations and well-child care

    • Infant/child stimulation for developmental progress
      o Nutrition

    • Signs and symptoms of illness and prevention of illnesses

    • Understanding of child’s behavior/psychosocial problems

    • Safety promotion and injury prevention, including car seat safety

Maternal Health:

  • Prenatal Education:

    • Fetal growth and development

    • Labor and delivery

    • Physiological changes

    • Early infant care

    • Health/nutrition/Family planning

    • Breastfeeding

  • Services after delivery:

    • Provide postpartum/newborn assessments

    • Teach infant care and feeding

    • Teach breastfeeding techniques

    • Teach birth control methods

    • Refer for medical follow-up as needed

  • Education/resources to pregnant and parenting teens

  • Education on maternal substance abuse

  • Grief and loss counseling and support for perinatal loss/Sudden Infant Death Syndrome (SIDS)

Project Connections Family Resource Center (FRC) - Health Access:

Public Health Nurses provide consultation to Family Resource Center Health Access Teams and home visitation to families referred to the FRCs. The Health Access goal is to link families to a health care home/immunization resource and funding for both. The Public Health Nurse and the Promotoras (health promoters) team up to provide in-home assessments, interventions and case management to meet family needs.
