Community Nursing Program

Nurse-Family Partnership (NFP) helps first-time parents succeed

The Orange County NFP program is the local site for the national Nurse-Family Partnership program, based in Denver, Colorado. Highly educated Public Health Nurses visit regularly during a 2½ year period, beginning in pregnancy. Families develop skills for parenting and economic self-sufficiency as they work with the nurse home visitor in the following areas:

  • Personal Health

  • Environmental Health

  • Maternal Role Development

  • Family & Friends

  • Life Course Development

  • Health and Human Services
     

Services Provided

Public health nursing services are provided in English, Spanish and Vietnamese. They include, but are not limited to, assessment, health teaching and referral, case management and community resources.

Our three main objectives are:

  • to foster healthier pregnancies

  • to improve the health and development of children

  • to encourage self-sufficiency

Home Visit Schedule

Once the client has been enrolled in the program, the nurse home visitor will visit:

  • Weekly for the first four weeks

  • Every other week until the child is born

  • Weekly, again, for six weeks

  • Every other week until the child is 21 months old

  • Monthly until the child is two years old

The Nurse-Family Partnership Program is based on the Prenatal and Early Childhood Model developed by David Olds, PhD.

Program Eligibility

Who is eligible to participate? Young women who are:

  • Under 24 years old, and

  • Less than 28 weeks pregnant*, and

  • First time pregnant and parenting, and

  • Eligible for Medi-Cal, CalOptima or emergency/pregnancy Medi-Cal, and

  • Desire to be involved in this program

(*The nurse must be able to make a home visit before the 28th week of pregnancy. Please refer as early in the pregnancy as possible.)

Referrals

The Nurse-Family Partnership receives referrals from a variety of sources, such as community and health clinics, private physicians offices, high schools, community-based organizations, Social Services Agency programs and Health Care Agency programs. Orange County Nurse-Family Partnership Information

Outcomes for NFP graduates:

  • 34% continued to work toward their diploma or General Education Development (GED)

  • 45% had earned their diploma/GED

  • 19% were pursing education beyond high school

The NFP enrolls women who are at higher risk for poor birth outcomes than the general population. Outcomes for the children of NFP graduates:

Birth Outcomes

  • 5% of NFP babies were premature, fewer than the OC rate of 9.8% (2002)
    and the CA rate of 10.2% (2002).

Immunizations

  • 90% or higher at 24 months of age compared to CA rate of 72% (2004).

Breastfeeding

  • 76% of NFP mothers initiated breastfeeding and 16% continued to breastfeed when their children were 12 months of age.

Emergency Room Visits

  • 82% of children had one or no visits to the ER by the age of 24 months, with the majority of those visits (83%) due to illness rather than injury or ingestion.
