The Senior Health Outreach and Prevention Program (SHOPP) provides services in the home to older adults with health care needs and behavioral health needs. Persons age 45 and above with unmet health care needs are eligible with priority given to frail older adults. Services are provided at no cost and include a visit from a Public Health Nurse who assesses needs and provides resources to help meet those needs.

  • Health and nutrition screening
  • Referral to Behavioral Health Services
  • Crisis and problem resolution
  • Case management
  • Client advocacy
  • Health and tobacco education
  • Linkage to health care and community resources

Behavioral Health Services*

  • Crisis intervention
  • Outreach and engagement
  • Brief mental health services including assessment, problem solving, advocacy and linkage to community resources

The SHOPP Staff

SHOPP services are provided by: Public Health Nurses, Clinical Social Workers, Behavioral Health Nurses, and a pharmacist.
