The Senior Health Outreach & Prevention
Program (SHOPP), funded by Tobacco Settlement Revenue, is a
collaborative effort between the County of Orange Public
Health and Behavioral Health Services. SHOPP provides
community outreach and home visitation to low income,
uninsured, or underserved adults with an unmet healthcare
need. Priority is given to frail older adults.
Outreach

Provide health education, outreach and direct
services to eligible adults.

Improve mental and physical health through
health screening, counseling, education, case management,
consultation, and client advocacy.

Improve access to health care through linkages
to community resources

Assure a supportive environment by
collaborating with a wide spectrum of community service
providers

Assist clients to identify and maximize their
strengths in daily living skills, health maintenance and
independent community functioning
The SHOPP Staff
SHOPP services are provided by: Public Health
Nurses, Clinical Social Workers, Behavioral Health Nurses, and
Pharmacists.
