Fees and Eligibility

Fees/Client Participation

Program services are free of charge. SHOPP is a voluntary program and clients may chose to start and stop participation at any time.

Eligibility criteria

Eligibility is based on the potential client’s ‘Unmet Health Care Need,’ according to specific priority guidelines, and need for Behavioral Health Assessment.

An unmet healthcare need is a medical condition, which impacts the individual’s ability to perform daily activities, access or obtain medical care, or comply with medical treatment or follow-up.

The individual may be isolated, unable or unwilling to access traditional services for their unmet healthcare need, and their health may continue to deteriorate without appropriate intervention and linkages to community resources and services.

All personal and medical information is confidential and will not be released except with written permission or by court order.
