Family-centered nursing assessment
Health Screening and counseling
Problem identification and intervention
Case management
Client advocacy
Preventive health teaching
Linkages to health care access and community resources and services.
Visit and assess clients with behavioral health issues in their home or wherever the client is most comfortable
Assess client’s functioning, as related to behavioral health, and determine strengths and needs of the client and/or family system
Assist in resolving crisis
Assist client and/or family to understand and work toward the solution of problems effecting behavioral health
Identify behavioral health resources and link client to community services
