Office of Care Coordination

The Office of Care Coordination engages across Orange County, working with cities and community-based organizations to strengthen regional capacity and multi-city, multi-sector investments to prevent and address homelessness, coordinate public and private resources to meet the needs of the homeless population in Orange County and promote integration of services throughout the community that improve the countywide response to homelessness.

ORANGE COUNTY SYSTEM OF CARE - AVAILABLE RESOURCES

Available resources refers to funding or allocations received by County departments that fund initiatives for which individuals and families experiencing homelessness are eligible. Some funding is considered to be renewed and used for continuing obligations, such as with some of the housing programs.

HCA OUTREACH AND ENGAGEMENT SERVICES

The Orange County Outreach & Engagement (OC O&E) program serves individuals experiencing unsheltered homelessness in Orange County. The program's services focus on linking individuals to needed housing, physical health, behavioral health, and other supportive services by addressing their barriers to care. This is accomplished through developing and building trusting relationships with the homeless community and collaborating with other service providers. Our multi-disciplinary team is proud to provide compassionate, client-centered, and consistent care to meet the needs of the most underserved people in our community.
