Program Description: PACT (Program for Assertive Community Treatment) is a field-based program for transitional age youth and young adults ages 14-21 living with a chronic and persistent mental illness, who may have a co-occurring substance use disorder, and who are experiencing significant impairments in their daily functioning. Outpatient services include the following intensive mental health services: assessment, individual, family and group therapy, substance abuse treatment, medication support, crisis intervention, extensive case management, and advocacy.

Serves: Orange County youth between the ages of 14-21 years old who have at least two psychiatric hospitalizations or one psychiatric hospitalization for at least 10 days and/or incarcerations, and are referred by their Children and Youth Behavioral Health (CYBH) clinician.

Hours of Operation: 8 a.m.– 5 p.m., Monday–Friday
