Project FOCUS
Program Description: Utilizing a “whatever it takes” community-based and client-centered approach where individualized care plans are developed for individuals and families, the Full Service Partnership Wraparound program of OCAPICA provides mental health services and intensive case management services. These services include support and assistance in obtaining benefits for low income families, health insurance, housing placements, parent education and support, tutoring, mentoring, youth recreation and leadership development.
Serving: Children, Youth and Transitional Age Youth, Age 0 to 26 who have serious emotional disturbances or severe mental illness with one or more of the following: homeless or at risk of homelessness, history of psychiatric hospitalizations, first psychotic episode, in danger of failing school, uninsured exiting Social Services Agency or probation system, unserved or underserved due to linguistic or cultural isolation, with special needs and/or co-occurring disorders, with parents who have serious mental illness.
Language(s): English, Vietnamese, Korean, Tagalog, Samoan, Cambodian, Chinese, Japanese, Hmong and other Asian Pacific Islander languages Hours of Operation: 9 a.m.–5:30 p.m., Monday–Friday. Project Focus provides crisis services for all enrolled clients 24-hours a day/7-days a week, as needed.
Project RENEW
Program Description: Project RENEW (Reaching Everyone Needing Effective Wrap) by Pathways of California serves Orange County children from birth to 18 years of age with serious emotional disturbance (SED)/serious mental illness (SMI), along with their families. The program focuses on children and their families who have been unserved or underserved, are and who may be homeless or at risk of homelessness. While the child is the identified participant, Project RENEW uses a Wraparound model, providing a team of individuals offering both formal and informal support to the entire family.

STAY
Program Description: Support Transitional Age Youth (STAY) of Pathways of California is a recovery based program for serious emotional disturbance (SED) and serious mentally ill (SMI) transition age youth (TAY), ages 16 through 25, who would benefit from increased integration into the community and support to assist in the development of the skills essential to becoming a self-sufficient adult. Founded with a vision that all TAY have hope, are empowered, can be responsible for themselves and be engaged in meaningful adult roles, the program supports participants in discovering and deciding what they want for themselves and how to develop the strengths, abilities and skills applicable to many aspects of their adult lives. Serves: Transitional Age Youth, Age 16-25, who are unserved or under-served, have experienced several psychiatric emergencies, may be exiting the Social Services system and/or are homeless or at risk of homelessness

CHOC Project Health
Program Description: Project HEALTH (Harnessing Every Ability for Lifelong Total Health) of CHOC is a Full Service Partnership wraparound program for children, teens and young adults with mental health and medical diagnoses that provides intensive in-home or community-based mental health and case management services to address critical gaps in psychosocial and health needs. Services include but are not limited to behavioral health and skills building, mental health services (individual, group, and family therapy), crisis intervention, referral to housing and food resources, developing of and referral to educational programs and employment skills, referral to legal assistance (guardianship, conservatorship, end-of-life issues), and referral to community resources and agencies.
