Comprehensive Health Assessment Team – Homeless (CHAT-H)

WHO WE ARE: The Comprehensive Health Assessment Team - Homeless (CHAT-H) is a public health nursing home visitation program serving residents of Orange County who are experiencing or at risk of homelessness and have health needs. The CHAT-H team works closely with Behavioral Health Services, BHS Outreach and Engagement, CalOptima, and Social Services. 

WHAT WE DO: CHAT-H’s goal is to engage individuals who have unmet health needs and help increase their access to services to address their health needs.  

HOW WE WORK: The CHAT-H team is multi-disciplinary and provides comprehensive case management and care coordination services, health education, referrals to resources for basic needs and community support services, and assistance in applying for Medi-Cal health insurance and accessing other Social Services and financial assistance programs.

 

CHAT-H staff can help with the following:

  • Medical and health concerns
  • Health Education
  • Connecting clients with:
    • Health Insurance
    • Emotional, health and substance use services
    • Food, basic needs and other resources
  • Arranging transportation to medical appointments

To make a referral to Community and Nursing Services, please complete the fillable PDF referral form and e-mail the completed form to publichealthnursing@ochca.com. Please e-mail form securely to protect PHI (ex. encrypted or password protected document).
