Public Health Nursing - Senior Health Outreach & Prevention Program (SHOPP)

The SENIOR HEALTH OUTREACH & PREVENTION PROGRAM (SHOPP) is a public health nursing home visitation program serving older adults in Orange County who have health care needs. The goal of SHOPP is to partner with individuals with unmet health needs by increasing access to services. The SHOPP team is multi-disciplinary and provides comprehensive case management, health education, referrals to community resources for basic needs, and assistance in accessing Social Services and financial assistance programs. The SHOPP team works closely with other county programs including Behavioral Health Services (BHS), CalOptima, and Social Services with shared goals in working with older adults.

Our staff can help with:

  • Connecting you with health services:
    • Medical, mental health, substance use, vision, hearing & more
  • Providing health assessment, managing health conditions & reviewing medications
  • Accessing health insurance 
  • Arranging transportation to medical appointments
  • Linking to community resources
  • Arranging for home delivered meals 
  • Finding a caregiver & supportive services
  • Maximizing client's strengths in daily living skills

How to Refer to SHOPP

To make a referral to Community and Nursing Services, please complete the fillable PDF referral form and e-mail the completed form to publichealthnursing@ochca.com. Please e-mail form securely to protect PHI (ex. encrypted or password protected document).
