Living with HIV

The 2018 Client Satisfaction Survey is here!

The Orange County Health Care Agency is doing a client satisfaction survey on Ryan White services in Orange County.  Please be honest and tell us 1) how satisfied you are with services; and 2) how we can improve services. This survey will help us to gather information about Ryan White services in Orange County.
  • Please take a few minutes to tell us about your experience in getting the services listed in this survey.  Only respond to services that you have received in the past 12 months.

  • Your answers will help make services better in the future.  Please do NOT give us your name.  Please complete this survey only one time.  Thank you.

  • Take the survey by clicking on this link: (Online survey only available in English. Paper copies of the Spanish survey will be available soon.)
