Effective January 1st, 2026, orders must be submitted through the OC Lab Web Portal. Handwritten and PDF-filled Lab Requisition Forms will not be accepted.
The OC PHL launched its Lab Web Portal, an electronic test ordering and results (ETOR) platform designed to replace paper lab requisition forms. The OC Lab Web Portal was designed to better serve our clients by providing near real time updates of their orders on a secure HIPAA compliant platform. It is an electronic ordering system, designed to reduce the paperwork submitted with orders, it is not paperless. Users can view, print, and download reports within the application.
Each facility must register an account with the OC Lab Web Portal before staff can create individual accounts.
To confirm registration or to register your facility:
1. Visit OC Lab Web Portal
2. Select New User Registration and check the organization list for your facility name
3. If your facility is not listed, please contact us (714) 834-8385 to have your facility account created
4. Select your facility name from the organization list
5. Create your individual user account
6. Registrations will be reviewed before accounts are activated
