About Community and Nursing Services (CNS)

WHO WE ARE: The Community and Nursing Services (CNS) division provides public health nursing services throughout the County, in homes and community settings, to individuals of all ages.  

WHAT WE DO: Nurses collaborate with community organizations to identify health concerns of populations, address needs, and provide health education and screening to help prevent disease and disability, promote healthy lifestyles, and assure access to quality health services. 

Nurses learn the client’s needs and help them access medical and social services, provide health education, and referrals for health care and community resources.

Services to Pregnant and Parenting Women and High-Risk Infants 

• Public Health Nursing services to pregnant or postpartum women and their infants
• Medically High-Risk Newborns (MHRN)
• Nurse-Family Partnership (NFP)®
• Perinatal and Infant Assessment and Coordination Team (PACT) 
• Grief and loss counseling and support for perinatal loss/Sudden Infant Death Syndrome (SIDS)
• Teen Parenting Programs

Services to Foster Youth

• Early Childhood System of Care (ECSOC)
• Emergency Response (ER) Public Health Nursing
• Foster Care Public Health Nursing Program

Services to Children, Adolescents, and Adults 

• Public Health Nursing services to children with special health care needs or who have chronic health conditions 
• Public Health Nursing services to adolescents and adults who need help obtaining medical care or managing a chronic health condition 
• Public Health Nursing services to homeless individuals – CHAT-H 
• SPECIAL SERVICES: Grief and loss counseling and support for perinatal loss/Sudden Infant Death Syndrome (SIDS)
• Senior Health Outreach & Prevention Program (SHOPP) is a public health nursing home visitation program serving older adults in Orange County who have health care needs.
